Case Management in Non-Government Alcohol and Other Drug Services; A Practical Toolkit

provides services with a suite of core-practice documents that have been modelled from best practice and validated stimulus from the local, national and international AOD platforms.

The utilisation of these core-practice documents across services aims to enhance the practice of case management in the NT Non-Government AOD Sector by providing more consistency in regards to processes, collaborative practices, skills and knowledge in the substance misuse field.

The toolkit contains three key resources for;

Understanding Case Management in Non-Government AOD services
Working within a Professional Framework
Case Management in non-government AOD services tools;

          Intake & Screening
          Assessment
          Care Planning
          Care Coordination

How to use the toolkit

The suite of practice documents in this toolkit provides case workers with a variety of information-gathering mechanisms, validated assessment tools and planning and coordination resources to effectively address the issues associated with their client’s problematic substance use.

It is within this context that services are in the best position to decide how the toolkit will complement or re-define their existing practices, noting that this toolkit is not a policy directive.
